1. Name of supplier

The Birth Clinic (a trading name of Premier Health Partners UK LLP No. OC348959) ('we' or 'us') shall be supplying the services to you.

2. What is to be provided to you by us

We and you have agreed that services will be delivered to you as described on the website, namely maternity information and access to care and testing through qualified medical practitioners at accredited or registered facilities. Unless otherwise agreed in writing, you agree to commencement in performing those services before the end of the seven day period following the day we enter into a contract.

3. Price

The price of the service you have requested is set out in http://www.thebirthclinic.co.uk/page/fees or via another form of communication if you are requesting something not outlined therein. The testing is not subject to VAT where the primary purpose of the service being provided relates to the protection; maintenance; or restoration of the health of the Patient. However, VAT may be charged (where applicable) for any other services.

4. When payment is to be made

You will typically need to make payment at the time you place your request. Please note that we will in most cases not be able to act on your request until we have received payment. With our prior agreement you may pay us, or the Practitioner with whom we engage, in cash at a time we appoint.

5. Cancellation rights

5.1 Once you have paid, we will have entered into a binding contract. You can cancel this contract within the seven day period that starts on the day after the contract is made, except that you cannot cancel the contract once services have been performed, or where you have agreed to perform the services before the end of the seven day period. Starting 7 days after the contract is entered into, you can only cancel by giving notice more than 48 hours ahead of your appointment. We will try to accomodate any changes to your appointment but cannot guarantee we will be successful.

5.2 To cancel the contract you will need to send a letter to us. You can send the letter by post, email or facsimile or by personal delivery. Contact details for where to send the letter are set out in Pa-ragraph below. If you cancel the contract orally, you will need to confirm the oral cancellation in writing and send it to us by one of the means just specified.
